	Dr. Lauterbach offers a free 15 to 20 minute initial consultation for prospective patients by appointment. Please contact Dr. Crew's office for more information.

	Fees for sessions are negotiated on a case by case basis with each individual patient considering variables such as time of day for a session, frequency of treatment sessions, length of session, services provided, and patient's financial situation.
	Sessions can be purchased in packages of 3, 5, or 10 at a reduced rate when paid for upfront.
	Cash and personal checks are accepted. *$20 fee for returned checks.

	Dr. Lauterbach is a provider for Medicare and the Empire Plan.
	Most other insurance carriers will pay for all or part of a session on an out-of-network benefit rate.
	Payment is expected at the end of each session, and a receipt will be provided for services rendered. Receipts can be used for insurance reimbursement from the insurance carrier directly to the patient. Dr. Lauterbach will assist in all necessary paperwork for out-of-network insurance reimbursement.
